New Patient Information
What to Expect on Your First Visit:
- Dr. Allen will personally give you a warm greeting.
- He will carefully listen to, and take a history of your concerns.
- He will perform a physical exam to find the cause of your problem.
- He may require additional lab or imaging (x-rays) which may be arranged nearby.
- He will review his findings and, if possible, outline a treatment program.
- If there are no countraindications he will initiate treatment.
- The visit will last about 1-1/2 hours and cost approximately $75.

My goal is to help as many individuals as possible obtain and maintain life-long health naturally through chiropractic adjustment, massage, nutrition, proper posture and exercise.
Key Points for Successful Care:
PATIENT SATISFACTION - I strive to provide outstanding service to my patients. Since results to chiropractic treatment vary, I can't guarantee results, but I can guarantee your satisfaction. Please let me know if there is any way I can improve my service and your satisfaction.
PUNCTUALITY - Please make every effort to be on time. A late arrival limits the amount of time I can spend with you and throws the schedule off for patients who follow.
CANCELLATIONS - Except in sudden emergencies, please give 24 HOURS NOTICE to cancel or reschedule. You will be billed for a missed appointment (a "no show"). Insurance companies will not pay for missed visits.
LENGTH OF SESSIONS - I believe strongly in the value of combining natural therapies to correct subluxations and restore health. Complete office visits include chiropractic treatment, therapeutic massage, proprioceptive and muscle energy techniques, stretching, physiotherapy, nutritional and life-style advice, and home exercise instruction. They typically last 20 to 40 minutes. The focus is always on achieving a treatment goal, rather than on a specific length of time.
TREATMENT PLANS - Treatment plans are designed to restore maximum possible function, not just relieve pain. Healing is a process that takes time. Each individual is unique. Because muscles and connective tissues are slow to heal, chiropractic care may be required for several months to more than a year. Everything will be explained as we go through the process of rebuilding a strong, healthy body. I will perform periodic re-examinations to provide you with road signs along the way to recovery and optimum health. I urge you to follow through with your treatment program.
COMMUNICATION - Open, honest communication is vital for the success of your care. Please speak with me if you have any concerns about your finances, progress or treatment plans before making any decisions.
PRIVACY - All patient records and conversations are private. Records are released only with your written permission. Please ask me for a privacy notice. We'll post it here soon.
ACTIVE CLIENT PARTICIPATION - Treatment will succeed to its fullest only if you take an active role. Follow through with recommendations for home exercise and lifestyle changes.

You may schedule an appointment with the receptionist or leave a voice mail message for me. I will call you back as soon as possible. If you have an emergency, I can be paged.
REFERRALS - My practice has grown mostly by word of mouth from satisfied patients. I believe that if I have provided superb service, my patients will tell others about the benefits of chiropractic care.

Financial Information
When paid at the time of service, the usual first chiropractic exam and visit is $95 and subsequent visits are $45. We accept most insurances, motor vehicle accidents and workers compensation cases. Please contact the office for details.
